Athletic Sign up / Registration
All of the following forms MUST be filled out and virtually signed by a parent or guardian BEFORE athletes can participate in the first practice. In addition, students must have a sports physical examination completed by a doctor and submit the sports physical documents to the athletic director or the coach.
